Teresa McIntosh born July 24, 1963 departed this life on Thursday – May 15, 2025 at the Hazard A.R.H. being 61-years old.  Teresa was born in Hazard, Kentucky the daughter of the late Matt Collins and the late Tilda Melton.  Other than her parents, Teresa was also preceded in death by her sister, Bernadine Combs.  Teresa was proudly a full time Nana, who loved spending every precious moment with her grandchildren.

Teresa McIntosh leaves the following relatives to cherish her memory: husband, Ben McIntosh of Hazard; son, Benny McIntosh & April of Hazard; daughter, Elizabeth Campbell & Buck of Hazard; brother, Matt Collins of Mousie; sisters, Regina Strong of Hazard, Vickie Goodson of Hazard, Melinda Collins of Mousie; 9-grandchildren, Brooklyn, Addison, Autumn, Tillie, Aubrey, Keely, Scooty, James and Stormy; also a host of other relatives & friends survive.

Visitation will be held on Monday – May 19, 2025 at the Engle-Bowling Funeral Home from 3:00 – 4:00 P.M.  Funeral Services will be held on Monday – May 19, 2025 at 4:00 P.M. at the Engle-Bowling Funeral Home with Lonnie Douglas Adams officiating.  The Interment will follow in the Creekside Cemetery located at Hazard, Kentucky.
